First Appointment

          

What to bring

For your convenience, please print and fill out the Patient Registration, Patient Medical History and Patient HIPPA Acknowledgement and Disclosure Consent forms and present them to the front desk when you arrive for your first visit.

In order to better serve you, we request that you provide the following items, some of which have been covered in the forms listed above:

  • A list of your current medications and supplements (vitamins). This list should include name, dosage and frequency of use for each medicine.

  • A list of medication allergies. This list should include any medicine to which your body has an abnormal (often uncomfortable) reaction.

  • A list of your past medical history.  This should include any prior or current major illnesses/health issues, a list of any major tests, especially if conducted within the last year and a list of any surgeries with at least approximate dates.

  • A list of your family's medical history. Important factors to include are history of heart disease, high blood pressure, high cholesterol, diabetes, or aneurysm.  Knowing when any of your close relatives passed away and cause of death is also important.

  • A list of your current health care providers. At a minimum include the name of each physician and the condition being treated. Additional helpful information would be address and telephone number of each provider.

  • A list of questions you have about your condition. This list should include your major concerns and should be kept at a reasonable length.

  • Any insurance cards for current coverage. 

  • Your driver's license or another valid photo ID.

Services

Your first appointment may last about an hour. Initially, a physician's assistant will gather preliminary data regarding your weight and vital signs. Questions will be asked about your health history and the reason for your visit. The physician will conduct a general physical exam and if necessary,  he may order additional tests. In order to facilitate the interactions between you and your cardiologist, it is best to come prepared. 

 

Telemedicine Visits

Telemedicine allows your healthcare provider to evaluate, diagnose and treat you remotely from the comfort of your own home using telecommunications technology via smart phone, mobile device or computer. After scheduling your appointment by phone, you will receive a text containing a link with information about your visit. When it is time for your appointment, you need to click on the link to be connected to Dr. Parto.

How to prepare

it is important to prepare for your appointment beforehand to make the most of your virtual visit.

  • Mobile device or laptop: Make sure your cell phone, tablet, laptop or desktop computer used for the video session is fully charged or plugged in. Check the strength of signal and internet connectivity of your device and adjust its sound quality

  • Quiet spot: Find a quiet spot for your appointment, free of distractions, interruptions and ambient background noise. Using earbuds or headphones may further reduce outside noise during your video connection.

  • Good lighting: Make sure there is sufficient lighting in that location, so the doctor can see you clearly.

  • Medical devices: Place the medical devices you have within reach. These may include a thermometer, blood pressure monitor, heart monitor, or even a bathroom scale. It is possible that the doctor may request a current reading while you are talking, and you don’t want to waste time retrieving these devices during the call. 

  • Symptoms list: Write a list of your symptoms and their duration beforehand to provide as much information as possible to the doctor. If you have a visible symptom, take a photo or be prepared to show it during your video session.

  • Medical history and medications: Have a list at hand of your medical history plus medications and supplements you are currently taking.

  • Questions: Draft your questions before the session and keep a notepad handy for recording the doctor's responses and instructions.
